Become a catalyst for change as the Mobile Shelter Support Program Case Manager with The Neighbourhood Group (TNG). Help adults in need navigate complex issues related to homelessness and health with targeted case management support.
This full-time position requires a case manager passionate about working with individuals facing diverse challenges in shelters and encampments across the GTA. You’ll provide one-on-one support, engaging clients through assessments, home visits, and resource coordination. Collaborate with community partners to create a integrated support system.
