Social Worker – RAAM Clinic – NADMIN 06/26

Job Summary:

Reporting to the Manager of the Mental Health and Addictions Program, Diabetes and Telemedicine, the Social Worker is responsible for coordinating, developing, implementing, and evaluating case management services within the RAAM Clinic. The role includes conducting staged screening and assessments, providing case

management and system navigation support for clients, facilitating group education sessions, and developing individualized treatment plans to support patients throughout all stages of their substance use recovery journey. The Social Worker also serves as an advocate for addictions services and harm reduction initiatives while promoting compassionate, client-centered care. This position requires a strong commitment to the values of Sioux Lookout Meno Ya Win Health Centre and the RAAM Clinic, including compassion, teamwork, respect, quality, inclusivity, accessibility, self-determination, and dignity.
