**Trinity Health Of New England** is looking for a **Licensed Social Worker** to join our Cancer Center Team.
The Social Worker LCSW will support our **Cancer Center** located **in Enfield, CT.** The LCSW will assess and assists in addressing and meeting the psycho-social needs of cancer patients and families, enabling patients to utilize medical care and other supportive services to achieve their optimum level of health and function.
**You Will Be Responsible For:**
+ Assesses and counsels’ cancer patients/families referred to the Cancer Center.
+ Provides consultation to physicians and staff in ambulatory discharge planning, from treatment, survivorship services, to psychosocial distress assessment.
