Detailed Overview

Reporting administratively to the Manager, Clinical Services and clinically to the Clinical Practice Leader, Social Worker, the Social Worker, Long Term Care uses professional and program standards in providing psycho‑social assessment, crisis intervention counselling and specific resident treatment, as well as assisting with tasks that are a cross‑responsibility of all team members.

Responsibilities

• Facilitates all admissions to the facility by liaising with Access, Home Health and clients; conducts admission interviews and tours; issues medical forms; completes required documentation; schedules admissions and conducts admission conferences for staff.
