Social Services Associate - Case Management

Description

Functions as a member of the PMG Care Management social work team and collaborates with the primary Care Manager LCSW/MSW to meet the social, emotional, financial and care needs of the patient. Provision of social services occurs within telephonically, MyChart correspondence, in PMG primary care clinic sites, Community sites and the home environment. Assists the primary Care Manager LCSW/MSW with individual skills training needs of identified patients.

Required Qualifications:

• Bachelor's Degree.
• 2 years Social Services experience.
