Senior Medical Officer (Home Care Services)

Responsibilities and Duties:

• Provide medical assessment and management for patients under Home Care Services and ambulatory care settings.
• Conduct home visits, teleconsultation reviews, and follow-up care where appropriate.
• Participate in multidisciplinary case discussions and transitional care planning.
• Maintain accurate and timely clinical documentation.
• Participate in quality improvement, audit, and service development activities.
• Support teaching and supervision of junior staff where applicable.
• Support continuity of care across outpatient, home care, nursing home, and community-based services where required.
• The doctor may be deployed, when required, across other clinical services, programmes, or care sites within St Luke’s Hospital and its affiliated/community partners, in accordance with organisational and service needs. This may include outpatient, community, transitional, or insti...
