Primary Focus

Reporting directly to the Supervisor of Complex Special Needs, the Resource Coordinator monitors the provision of services for eligible children. Using a person‑centered approach, the Coordinator oversees the development and implementation of evidence‑based service plans, coordinates services on behalf of the individual, and is responsible for reassessment and evaluation of current services and supports, as well as future options.

Main Responsibilities

• Engages with the individual and their family, assesses current and future options, provides resource links, and participates in setting goals through the Plan of Care and Individual Support Plan.
