Registered Nurse - Transition Graduate Nurse

The Transition Graduate Nurse (TGN) is a regulated health professional who provides comprehensive nursing care to individuals and families across the lifespan. The position is supernumerary and not part of the baseline staff count. The TGN works with a Clinical Guide, providing care until ready to independently assume the full scope of RN or RPN practice. Upon readiness, the TGN secures a regular position of no less than 0.5 FTE and continues to work the same total FTE as their original GNTPP role, with only the new position FTE counted in the baseline staff complement. The TGN conducts holistic assessments, develops and implements care plans, coordinates interdisciplinary care, and contributes to health system improvement across diverse settings – acute care, continuing care, community health, and specialty programs.
