Registered Nurse (Transfer Coordinator)

Summary The Registered Nurse Transfer coordinator is directly responsible for the facilitation and monitoring of the community hospital referrals to the VA facility, VA to VA, and VA to community hospital. This individual is substantially responsible for collaborating with other staff for appropriate oversight and management of emergency care received through community hospitals. Responsibilities Duties and responsibilities, but not limited to: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex client care.
