Hours of Work: Site open 7 days per week; must be available for Day/Evening 8 & 12 hour shifts
Location: Humber River Health, Church site
Position Responsibilities
- All aspects of care for patients receiving hemodialysis with a primary nursing focus
- Providing education and follow‑up to all hemodialysis patients
- Providing primary care to assigned patients, ensuring a plan of care is developed in collaboration with the multidisciplinary team, updated and communicated with the team in a timely manner
