Summary The Marion VA Health Care System is seeking a full-time Registered Nurse to serve as the Contract Nursing Home (CNH) Liaison within Care Management & Social Work service located in Marion, IL. The CNH Liaison is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Responsibilities The CNH Liaison is responsible for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The CNH Liaison is responsible for providing competent evidence-based care to assigned patients. Provides management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. The CNH Liaison coordinates care with multiple providers across all levels and sites of care.
