PSW, Palliative Care

This is a unique opportunity to be part of a tight-knit collaborative team, you will assist palliative patients with personal care and activities of daily living. You will support patients, families and caregivers –enhancing quality of life, comfort, dignity and security in patients’ end of life journey. All direct patient care activities will be performed under the direction of an Integrated Care Program Supervisor who is a nurse. 

• Working alongside the interdisciplinary palliative care team, you will provide specialized care as specified in the care plan, according to protocols, within scope of practice and in accordance with Spectrum Health Care’s policies and procedures.
• Provide companionship and comfort, creating a calm and respectful environment tailored to the client's needs, preferences, and goals of care.
