Humana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands.
This is a full-time, on-site, patient-facing role. You report to the Associate Medical Director and are responsible for both in-visit care and ongoing patient management.
**What You'll Do**
+ You will see 12-15 patients per day, allowing for longer, more in-depth visits
+ You will manage patients with complex and chronic conditions
+ You will complete accurate, same-day documentation to support patient care and outcomes
+ You will manage a daily clinical inbox (labs, messages, follow-ups)
+ You will coordinate care with specialists and care team members
+ You will apply Medicare risk adjustment principles through accurate condition documentation
