POSITION OBJECTIVE
As a key member of the interdisciplinary health care team, you will apply your critical thinking skills and knowledge of drug therapy in the provision of safe and effective medication-related care for patients of Niagara Health. Responsibilities will include, but are not limited to, addressing patient-specific drug related problems, collaborating with prescribers in the optimization of medications, clinical review and processing of prescriber orders, supporting the Medication Reconciliation process, engaging in various quality improvement/safety initiatives or projects.
