Patient Care Manager, Acute Medicine/Integrated Care

DUTIES AND RESPONSIBILITIES:

The Patient Care Manager (PCM), Medicine is responsible for the management of the clinical service area and providing visible leadership to the interdisciplinary team to ensure optimal performance management and smooth operation of the service area and high quality patient care. As a transformational leader, the PCM will create a vision to guide positive change and innovation through inspiration, motivation, role modelling and coaching and by working in collaboration with the leadership and Inter-professional teams in the Medicine Program. The PCM facilitates and supports implementation of evidence based practice to achieve excellence in high quality delivery of patient care.
