Outreach Care Specialist Lead - Carelon

Position in the Carelon Care Coordination team dedicated to improving member health outcomes.

Location & Work Arrangement

Vancouver, Clark County, Klickitat County, and Skamania County, Washington. Hybrid work model: 1-2 days per week in-office, remaining days remote.

Responsibilities

• Coordinate follow-up care plan needs for members by scheduling appointments or enrolling them in programs.
• Assess member compliance with medical treatment plans via telephone or on-site visits.
• Identify barriers to plan compliance and coordinate resolutions.
• Identify opportunities that impact quality goals and recommend process improvements.
• Recommend treatment plan modifications and determine need for additional services in collaboration with case management and providers.
• Coordinate identification of and referral to local, state, or federally funded programs.
