As part of the Home and Community Care team, you'll engage with clients and families to enhance their daily living through strategic assessments and interventions. Your role will focus on functional mobility, cognitive health, and community resource access, fostering independence and well-being across the Edmonton Zone.
Key Responsibilities:
• Conduct comprehensive assessments for physical and cognitive needs
• Develop and implement personalized treatment plans
• Use OT models to support rehabilitation goals
