Occupational Therapist I

This position supports clients and the Home Care Team in both Chestermere & Airdrie. If you are interested in working within an interprofessional team playing a key role in linking the needs of Home Care clients with appropriate health and community services, then this position will be of interest to you. Based on the Continuing Care Case Management Competency Framework, you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person‑centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self‑care goals and provide direct care.
