Nurse Practitioner – Primary Care

ConnectWell Community Health is a fully accredited, community governed, and not‑for‑profit organization. ConnectWell provides community-based primary care, health promotion programs and services, and a range of developmental and social services across Lanark, Renfrew, and Leeds and Grenville Communities. ConnectWell Community Health services are provided by salaried interdisciplinary team members, working to their fullest possible scope, in a collaborative practice, to address the health and social well‑being to residences. A strong understanding and willingness to learn about social determinants of health needs of the people and communities we serve.
