In this role, you will report to the Clinical Director within the Renal Program. As a Registered Nurse in the Extended Class, you'll manage chronic kidney disease patients while collaborating with nephrologists and interdisciplinary teams. Your expertise is vital for seamless transitions and delivering comprehensive care to enhance patient health.
Key Responsibilities:
• Formulate clinical decisions regarding CKD patient management
• Collaborate with other Renal Nurse Practitioners
• Participate in interdisciplinary meetings to support patient care
• Act as a key liaison with primary care and community partners
• Provide expert nursing consultation related to CKD
