In this role, you will deliver client-centered care to individuals of all ages at the College and Danforth Clinic. As an essential part of the Primary Care Team, you will manage your caseload, perform assessments, and ensure continuity of care. Your work will focus on addressing health disparities, especially for immigrants, refugees, and 2SLGBTQI+ populations.
