Nurse (N2) - Primary Care

Position Overview

Consistent with the principles of primary healthcare, this role provides access to first‑level basic healthcare within the scope of nursing practice for individuals, families, groups, and communities. As a member of an interdisciplinary community area team, the Primary Care Nurse delivers comprehensive care with an emphasis on healthy living, illness prevention, health education, chronic disease management, clinical intervention, and palliation. The nurse develops and implements health plans with clients, evaluates outcomes, coordinates ongoing services, and links clients to community resources.
