About the Role
Rexdale Community Health Centre is a non‑profit, multi‑service facility that offers primary health care, settlement, and community development activities for women, families, youth, adults, and seniors from diverse cultural backgrounds. The Centre is dedicated to delivering accessible and sustainable programs and services to diverse communities. It employs service delivery methods that incorporate advanced access principles, utilizing group visits and self‑management models.
Currently, the Centre is seeking a Mental Health Case Manager to collaborate with the mental health team in providing support and primary health care to Black children, youth, and mothers. The successful candidate will join the Centre’s clinical team.
Responsibilities
- Coordinate the integration of social services/case management functions into the client care with other departments, external service organizations, agencies and healthcare facilities.
