Job Profile

As Program Coordinator for the Housing First Program, you will collaborate with the Manager to coordinate mental health, housing, and financial support services, manage client referrals, coordinate with third parties, and provide direct mental health supports to housing clients.

Responsibilities

• Monitor incoming referrals, community outreach, and workshop/event requests.
• Assist with coordination of community events approved by the Manager.
• Consult with the Manager to determine program capacity for workshop and group requests.
• Receive and confirm new referrals, conduct initial needs assessments, enter and document client activity in CRMS and HIFIS, and route clients to appropriate services within the Housing Programs.
