This Pharmacist I position offers the opportunity to work collaboratively within an interdisciplinary team that includes infectious disease physicians, nurse practitioners, nurses, social workers, dietitians, and clinical pharmacists at the Northern Alberta Program (NAP) at the Kaye Edmonton Clinic.NAP provides comprehensive assessment, treatment, education, and support for individuals living with HIV in an ambulatory care setting, including access to antiretroviral therapy. Practicing to full scope, the pharmacist is responsible for reviewing and optimizing medication therapy by identifying and resolving actual or potential drug-related problems to improve patient outcomes. This involves active collaboration with the healthcare team and building strong, patient-centered relationships to develop individualized treatment plans.
