BayCare HomeCare is currently in search of our newest Home Health, Physical Therapist Case Manager, who is passionate about providing outstanding customer service to our community.
+ 📍 Locations: **Hillsborough, Manatee, Pasco, Pinellas, Polk, and Sarasota** counties
+ **Facility:** BayCare HomeCare
+ **Status:** Full time, Salary
+ **Shift Hours:** 8:00 AM - 5:00 PM
+ **Weekend Work:** Occasional
+ **On Call:** No
