Facilitate communication and collaboration among nurses/participants/staff for our drop-in programsEstablish and coordinate relationships with other healthcare services/external agencies for program development and implementationCoordinate care plans and services with external partnersResearch and identify potential resources and grants for health carePrepare and submit grant proposals to secure funding for healthcare programsAdhere to PHIA/FIPPA regulations and maintain strict confidentiality of participant informationFacilitating Health Programs: Engaging and Presenting to ParticipantsOrganize and maintain participants’ medications, ensuring all prescriptions are current and accurately documented on MAR (Medication Administration Record) sheets.
