Key Responsibilities:
• Develop and implement behavioral support plans for clients
• Facilitate community integration and transportation
• Conduct assessments and monitor intervention success

Requirements:
• Degree in health-related discipline required
• At least three years of relevant community experience
• Certification in First Aid and CPR preferred
