An outpatient primary care organization is seeking a Spanish-speaking Nurse Practitioner to join a value-based clinic in Tucson, Arizona.

This clinic focuses on providing preventative and chronic care management for Medicare patients through a collaborative team model and smaller patient panels.

The care model prioritizes patient outcomes, preventative medicine, and stronger provider-patient relationships.

• Schedule: Monday Friday
• Hours: 8:00 AM 5:00 PM
• Setting: Outpatient primary care clinic
• Patient population: Primarily geriatric / Medicare
• Visit length: 30 minutes
• Patient panel capped around 450
• EMR: Epic
