Empower patients through diabetes education as a Registered Nurse at Waterloo Regional Health Network (WRHN). This part-time role focuses on effective patient-centered education and clinical monitoring.
As a Diabetes Educator, you will report to the Manager of Ambulatory Care at the Adult Diabetes Education Centre. Your expertise in diabetes management will enhance patient education for those both in outpatient and inpatient settings. With over five years of experience and a passion for community health, you'll contribute significantly to patient care and support.
Key Responsibilities:
• Provide effective diabetes education for patients
• Conduct clinical monitoring within an outpatient setting
• Support admitted patients needing diabetes education
• Facilitate group sessions and team interactions
• Mentor and coach staff on diabetes care practices
