Mon-Fri either 8am-5pm or 7:30am-4:30pm - hybrid with in-person patient meetings at our SWLR Clinic - Bachelor's & 1 year of work exp in related fields required - Bilingual strongly preferred
**Salary:**
Most new hires start between $20.50-$25.60 per hour, depending on experience and qualifications.
**Additional Information:**
The Community Health Worker (CHW) provides coordination of care for patients and families between ACH primary care providers and appropriate community resources. The CHW will help families navigate various systems of care while connecting them to the medical, social, and mental health services for which they may be eligible.
