Community Health Representative

Reporting to the Health Services Manager and working with the Community Health Nurse, the Community Health Representative promotes the health and well‑being of Zagim Anishinabk members through health education, prevention, outreach, and community support.

Duties and Responsibilities

• Plan and deliver community health education, awareness, and prevention programs.
• Coordinate and administer the Children’s Oral Health Initiative.
• Conduct home and hospital visits for seniors, individuals with disabilities, and chronic care clients.
• Connect community members with health programs and provide follow‑up support.
• Assist the Community Health Nurse with monitoring, follow‑up education, and regarding communicable diseases.
• Support public health education at Goose Lake School and Zagim Daycare/Headstart.
