Key Responsibilities:
• Develop client care plans with health care teams
• Provide assessments for new and existing clients
• Facilitate training programs for Home Support Workers
• Maintain client records and prepare monthly reports
• Act as a client advocate for community resources
Requirements:
• Bachelor of Nursing Degree plus 2 years experience
• Valid CPR certification or ability to obtain
