Community Connector

Northern Medical Services (NMS), a division of the College of Medicine at the University of Saskatchewan, works in a unique, geographically and culturally distinct setting to provide equitable and accessible health care in northern Saskatchewan.

Primary Purpose

The Community Connector (Social Prescriber) works as part of the La Ronge Medical Clinic’s interdisciplinary care team to support and advocate for patients’ social, cultural, and practical needs that promote health and well‑being. The role connects individuals and families with community‑based resources, programs, and supports, reduces barriers to care, improves patient self‑management, and enhances health outcomes. It also supports partnership building with community organisations and advances social prescribing initiatives.
