Clinical Provider Engagement Partner – RN or Case Manager

Responsibilities

• Build trusted, lasting relationships with PCPs, care coordinators, office managers, and clinical leadership, becoming a known and valued presence in every practice you serve.
• Help providers understand exactly how Porter’s in‑home NP assessments support their patients, closing care gaps, surfacing undiagnosed conditions, and improving the quality of information available at the next visit.
• Show providers the tangible impact Porter has on the patients they share, including better documented complexity, more comprehensive visits, and members who show up more prepared and more engaged.
• Collaborate with provider offices to identify shared members who could benefit most from Porter’s services, and support seamless coordination between Porter’s clinical team and the practice.
