Amergis Healthcare Staffing is partnering with a leading Adult Day Health program to hire a compassionate and experienced **LPN Case Manager** to support elderly adults and individuals with disabilities throughout the Worcester area. In this community-based role, you'll conduct in-home assessments, monitor client health and well-being, collaborate with caregivers and healthcare providers, and help coordinate services that promote independence and quality of life.
This position is ideal for an LPN who enjoys building relationships, working independently in the field, and making a meaningful impact through personalized care management.
**Key Qualifications:**
+ Active Massachusetts LPN license in good standing
