Care Partner - Aged Care

As a Care Partner, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.

To best serve our diverse clientele, we are seeking a Bilingual Care Partner with native or professional fluency in English and either Cantonese or Mandarin.

This role is full time, virtual and hybrid, working from home and our Castle Hill office.
