**Job Summary:**
Under the supervision of the Manager, Integrated Health Care Management, the Senior Case Manager is responsible for assisting the Manager in the daily operation of the Integrated Health Care Management department by precepting and mentoring new and existing associates, and participating in workflow development, training and implementation and auditing. In addition, the Senior Care Manager provides individual care management services for members including: identification of members appropriate for care management, assessment of barriers, strengths and needs; development of an interdisciplinary person-centered plan of care; monitoring and evaluation of the member’s progress towards goal; and initiation of plan of care revisions, as needed.
