About the Role

As ACC Clinical Support Coordinator , you will support the day-to-day ACC administrative and coordination requirements to prepare for discharge and transition to the community.

You’ll need to ensure patient information is entered correctly and accurately into the system and respond to queries in a professional, timely manner.
