Salary

Salary Range: $31.67 to $40.12 per hour

Position Overview

Under the direction of the Supportive Housing Manager, the Case Manager acts as clinical lead and one of the senior practitioners of the Supportive Housing 24‑hour care setting that is consistent with the vision, mission, and core values of Dilico Anishinabek Family Care. The Case Manager will be primarily responsible for engagement and stabilization with a strong focus on transition and aftercare planning.

• Provide clinical consultation and guidance to the Supportive Housing program team.
• Ensure continued integration with other internal programs and community partners.
• Incorporate cultural aspects into the stabilization plan and supportive housing environment.
